Media Release Consent

[ ] Option 1: Full Grant (Yes): I give permission to use my child's photograph, video, and name in all school media (websites, social media, print brochures, and newspapers).

[ ] Option 2: Limited Grant (Digital Only): I give permission to use my child's photograph or video on the secure school portal and official website, but not on public social media (like Facebook or Instagram).

[ ] Option 3: Limited Grant (No Name): I give permission to use my child's photograph or video, but never publish their name alongside the media.

[ ] Option 4: Deny Consent (No): I do not give permission for my child's image or video to be used in any school promotional materials.

I understand that neither the student nor the parent/guardian will receive any payment, compensation, or other financial benefit for photographs or recordings taken or used by the school.

I understand that I may withdraw this permission at any time by submitting a written request to the school’s front office. Revocation will apply to future use of photographs or recordings and may not affect materials that have already been published or distributed.

Photographs and recordings created by authorized school staff in connection with school activities remain the property of the school, subject to applicable laws and the permissions granted under this form.
